The Rise of JUUL E-Cigarette Use
Electronic nicotine delivery systems (ENDS) commonly are referred to as e-cigarettes, although they have many names, including vapes, e-cigs, and mods. Because of the popularity of the JUUL brand, JUULing is a term commonly associated with the use of the products.
JUUL e-cigs were originally marketed to promote smoking cessation. However, the marketing strategies for the products—which promote these products as being safe—along with the appeal of flavors and social influence has had an effect on kids and young adults who use an e-cig more often than traditional cigarettes.
While e-cigarettes were originally claimed to be safer than traditional cigarettes, further analysis into the original research that claimed they were 95% safer is not clear or fully comprehensive. While a study by the National Academies of Sciences, Engineering, and Medicine did find that the exposure to toxic substances through e-cigarettes was lower compared to combustible cigarettes, it now appears that e-cigs like JUUL have their own unique health risks.
It was for this reason that in 2018 the U.S. surgeon general issued an advisory about the epidemic of e-cigarette use among America’s youth.
This advisory led to the most recent ban of mint- and fruit-flavored vaping products. The flavoring ban does not apply to menthol and tobacco-flavored products. Vaping shops are also allowed to sell flavors from tank-based systems, which allow people to mix their own vape juice.
The action by the FDA came as a response to the rise in vaping among and a vaping-related lung injury epidemic that has resulted in 55 deaths and 2,561 people hospitalized.
JUUL use, specifically, has increased in recent years, especially among teens and young adults, who prefer using pods and cartridges to tank systems found in vape shops.
Because the JUUL looks like a flash drive and is discreet, it is easy for teens to hide at school or home. These e-cigs are also highly addictive. There is 0.7 mL of nicotine per pod, the equivalent to one pack of cigarettes, so each pod packs quite the nicotine punch.

JUUL Injuries and E-Cig Side Effects
Researchers at Stanford School of Medicine released a study in May 2019 that suggested JUUL e-cigarettes and other vapes could increase the likelihood of heart disease. They found that endothelial cells exposed to e-liquid exhibited DNA damage or cellular death, which even occurred in instances where nicotine was absent.
Another study found that people who use e-cigarettes like JUUL are 56% more likely to have a heart attack and 30% more likely to have a stroke.
The notion that JUUL e-cigs are just water vapor, nicotine, and flavors is untrue. The vapor contains harmful chemicals, heavy metals, and ultrafine particles. For example, e-cigarette products made by JUUL Labs were contaminated with a microbial toxin that can cause long-term lung damage.
Researchers from Harvard conducted a study where they analyzed 54 JUUL pods for two types of microbial toxins: endotoxin and glucan, and 46% of the samples contained detectable levels of glucan, with tobacco and menthol being much more contaminated. Chronic exposure to glucan can cause inflammation to the airway and result in long-term lung damage.
The American Cancer Society reports that, in addition to nicotine, e-cigarettes contain propylene glycol and/or vegetable glycerin, which increases the likelihood of lung or airway irritation. E-cigarettes may also contain:
- Volatile organic compounds (VOCs): These can cause eye, nose, and throat irritation and damage the liver, kidney, and nervous system.
- Formaldehyde: This substance can cause cancer from overheating.
- Flavoring compounds: Some flavors contain different levels of the chemical diacetyl with links to bronchiolitis obliterans, a serious lung disease.
While scientists are still learning how an e-cigarette can affect health when used for long periods of time, chronic inhalation is shown to:
- Induce asthma
- Decrease lung function
- Cause respiratory cancer
